FAQ’s for Insurance Processing

Do you take insurance?

We are an out-of-network provider, so we ask for payment up front. We will contact your insurance provider and let you know what benefits are covered. Our detailed invoice with insurance codes provided gives you the information you need to file with your insurance company. Instead of investing a great deal of time and money in structuring Optimal Health to be insurance driven, we would rather pass the savings and health benefits onto you.

How come you do not handle the processing?

We believe our commitment is to you and your health, not the insurance companies. If we allowed the insurance companies to dictate your care, we would be providing a disservice to you. We also believe in choice heath care. What this means is that Optimal Health and Wellness takes a multifaceted, natural approach to health, wellness and anti-aging.

How do I get my benefits covered?

We will provide you detailed statements that you can easily file with your insurance provider, so reimbursement runs smoothly. We have found our clients appreciate this approach, because you have the opportunity to negotiate benefits and may obtain more reimbursement by handling it yourself instead of going through us.

Am I expected to submit my statements every time I come here?

You can pay for a pre-care plan which is a recommended series of treatments that you can pay up-front for a discounted fee. You can then file one statement and insurance will determine reimbursement before care is completed. This allows for a piece of mind knowing that you can visit us when needed and not pay additional costs. You also get the discount benefit.

I have never filed my medical statements and would have no idea how to do this?

It is not complicated at all. The number on the back of your insurance card is for you to call and ask how to submit a claim for payment. They usually have these forms on-line and are very easy to complete. The process can take about 10 minutes to complete, in some cases longer.

Types of Payment Accepted

 We offer several methods of payment for your chiropractic care at our office. You may choose the plan which best fits your needs. Please read over the options carefully and choose the plan you prefer. This information enables us to better serve you. Our main concern is your health and well-being.

 A. CASH (Includes checks)
 1. Payment in full is expected at the time services are rendered.
 2. You may make a single payment in full at the end of the week for multiple visits

      during the week.

 
B. MAJOR CREDIT CARDS (Includes MasterCard, or Visa)
 1. Payment in full is expected at the time services are rendered.
 2. You may make a single payment in full at the end of the week for multiple visits during

      the week.

 
 C. CARE CREDIT
  Care Credit is a convenient payment plan for those who need to spread the cost of

 treatment over time.

 
  With care credit:
 • There are flexible financing options
 • A credit decision usually only takes a few minutes
 • There are no annual fees or prepayment penalties

 • Pre-payment plan participation required 

 • You can make monthly payments as low as 3% of the outstanding balance.
 • 3, 6 & 12 month no interest plans available for those who qualify
 • 9.9% interest on Extended Payment Plans 24 to 60 Months

 D. OUT OF NETWORK INSURANCE COVERAGE

  We honor the fact that you work hard and pay dearly for your insurance, and we are more than happy to provide you with a receipt including any codes which you may use to submit to your insurance on your own. However, please understand that if you are entitled to any reimbursement under your individual policy it will be considered out of network and will come from your insurer directly to you. As a courtesy, we would be happy to call on your behalf to find out any out-of-network insurance coverage you may have.
